WHAT KIND OF INFORMATION DO I NEED TO COMPLETE THESE DOCUMENTS?

• Your name, date of birth, address, phone number, doctor's phone number

• Two emergency contact names & phone numbers

• Your signature OR parent/guardian signature for youth under age 18

WHO NEEDS TO FILL OUT WHICH DOCUMENTS?

Every person who sails on a boat at Sail Sand Point (including one-time guests of regular Open Sailors) must complete a Medical/Liability Waiver for the current year.

Every student who takes a class or lesson must complete a Medical/Liability Waiver and a Boat Use Agreement for the current year.

Every sailor who possesses a green card for Open Sailing must complete the green card, a Medical/Liability Waiver, and a Boat Use Agreement for the current year.

Every person who is under the age of 18 must have all documents (Medical/Liability Waiver, Boat Use Agreement, and/or Open Sailing green card) signed by a parent or legal guardian.
